Manage the sharing of your health information

Universal Authorization

Geisinger takes steps to limit the sharing of certain sensitive information without your consent. This includes inpatient psychiatric records, substance use disorder records, and information regarding your HIV/AIDs status. If you have received these services, sharing this information is vital to coordinating your care.

If you want to share this sensitive information in accordance with HIPAA, we ask that you sign a Universal Authorization. A Universal Authorization may be revoked at any time. 

MyGeisinger Users

Complete the Universal Authorization online.

You can sign a Universal Authorization during registration at any visit, sign into MyGeisinger, or you may print the form using the button below and return it to:

To revoke a previously signed Universal Authorization, you may use our revocation form or discuss the issues with our registration staff at your next visit.

Health information exchanges (HIEs)

To promote coordinated care, Geisinger makes your medical records available to doctors and hospitals that participate in HIEs in which Geisinger is a member. This provides continuity of care and reduces the need for you to supply paper copies of records to your doctors.

If you do not wish to participate in HIEs, you may use our HIE opt-out form, or discuss the issues with our Registration staff at your next visit.

Privacy Practices Acknowledgement

Effective Oct. 1, 2019, Geisinger has made important changes to our Notice of Privacy Practices. At your next visit, you will be asked to sign a new Privacy Acknowledgement
