May 7, 2009, DANVILLE – Geisinger care management program’s co-directors recently presented at the nation’s largest annual conference of hospital/healthsystem care managers.
The 16th National Institute of Case Management conference held in conjunction with the 10th American Case Management Association meeting in Boston, April 18- 22, featured more than 50 workshops and presentations for approximately nearly 800 nurses, social work case managers, medical directors, physicians, and hospital administrators from the United States and Canada.
Susan Standish-Wallace, RN, MS and Lisa Golden, RN, presented “The Pendulum Swings, Reducing Readmission Rates Within 30 Days of Discharge” which addresses their ongoing efforts to advance patient care beyond the acute care hospital.
