From Route 322, take the Mifflintown exit and go South onto Route 35. After the exit, still going south on Route 35, you will come to a stop light. Go straight at this light and proceed across the Juniata River bridge. Staying on Route 35 South you will wind through the town of Mifflin and then go across a railroad bridge. After this bridge, staying on Route 35 South, the road will take a natural turn to the left and you will go up a hill. When you begin to go down the hill, about 1/4 mile on the left you will see a sign for the Geisinger clinic. Turn left into the driveway, then make the first right, The clinic is on top of the hill; it is a one-story red brick building. Geisinger's Skilled Nursing Facility (SNF) Program, staffed by CPSL physicians, nurse practitioners and physician assistants, provides both acute and transitional care to patients living in area nursing homes, enhancing the care they already receive from nursing home staff. The SNF Program streamlines patient transition from physician offices or hospitals to the nursing home and improves patient outcomes to reduce the need for hospitalization, working closely with nursing home staff to coordinate and improve patient care.
